Questionnaire for the community

Our church is keen to find out from those who live in its local community whether there are any ways in which we could help you, especially with some of the challenges that you face in your family – whether it’s young children, elderly parents or anything in between. We’d love to hear from as many people as possible, so, if you are able to return this form with your ideas, we’ll be very grateful.

What, in your view, are the most important needs in this community?
Which of these needs are not being met by anyone at the moment, as far as you know?
Are there ways in which you would like some help in your family life that is not available to you at the moment?
